Understandings For Use   (initial each item)

1.  No Smoking Anywhere in Building.  __________

 

2.  No Alcoholic Beverages on the Premises.  NO EXCEPTIONS  __________

 

3.  Payment in Full of $200.00/day & $100.00 Refundable Deposit are
Due At Board Meeting.  __________  Minimum of 3 WEEK Cancellation Notice is
Required or Renter Forfeits $200.00 Use fee.  __________  Rental Fee is for Building
and Grounds ONLY.  Does not Include Plates/Napkins/Paper Towels Etc.  Renter
MUST Provide Their Own.  __________

 

4.  ONLY LOCAL Phone Calls May Be Made from the Club House Phone
at 25 Cents per Call.  Place Money in the Small Box Under The Phone.  __________

 

5.   When Finished User Agrees To ALL of the Following:

a.  Return ALL Tables & Chairs to the Racks.  ________

b.   Leave Heat at 55 degrees or A/C at 80 degrees.  ________

c.  Leave Club House and Grounds Neat and Clean.  ________

d.   Turn OFF ALL LIGHTS & Lock All Windows and Doors.  ________

e.  Return Keys to Club House Representative within 24 Hours.  ________

 

6.   Report ANY & ALL Club House Problems to the Lions Club Representative.  __________

 

7.   User IS RESPONSIBLE for ALL DAMAGES, if any, Incurred During the Rental. 
Your Deposit Will Be Refunded After Satisfactory Inspection
By a Lions Club Representative.  __________
